Assistance for participants with complex disability support needs to navigate the National Disability Insurance Scheme (NDIS) and to work effectively with stakeholders and people in your life. We work from a strengths perspective to identify your needs, personalise your supports and make sure you are matched with the right services. We work closely with your supports, friends and family to build capacity, to understand and implement your plan, to coordinate your supports and strengthen your informal networks. At The Way Forward, Specialist Support Coordination is delivered by Amy Gammon, who is a social worker with more than a decade’s experience in supporting people with disabilities using a specialist framework, Some of the ways we night help you include:
• Coordination: We will coordinate complex support and services which are developed and delivered within your NDIS plan
• Support Design: We will develop and design support solutions to meet the outcomes you are hoping to achieve, all while considering the risks in your situation and what you may need to stay safe. We will work with you and the people in your life to achieve your goals through building capacity, knowledge, resourcefulness and confidence
• Establish Supports: We will assist you and people in your life to identify and consider support options and facilitate engagement with broader systems of supports, such as health, justice and education services. We help you to create a support plan to facilitate the goals in your NDIS plan.
• Stakeholder Engagement: We will maintain effective transparent relationships and communication with internal and external providers and allied health care professionals. Where needed we will work with you to build strong relationships
• Crisis: Assistance to resolve points of crisis and developing capacity and resilience in your network.
• Coach, Refine, Reflect: Coach you through challenges that come up and assist you to prepare for review and report on achieved outcomes on a regular basis to the NDIA
